Job Description:
Makes the initial nursing evaluation visit.
Continually evaluates the patient’s nursing needs.
Initiates and coordinates the plan of treatment and necessary revisions.
Renders nursing care and performs treatments as ordered by the physician.
Reports to the patient’s physician and other appropriate personnel pertinent changes in the patient’s condition.
Supervises and teaches family and personnel.
Participates in nursing report.
Responsible for patient recertification for home care services.
Participates in QA/QI process.
Prepares reports and documentation.
Is responsible for adhering to all practice standards as they apply to patient care.
Job Requirements:
Current RN licensure in state of employment.
At a minimum, one (1) year clinical experience. This requirement may be waived for an RN with recent clinical experience as an LPN.
Current driver’s license and vehicle insurance in state of employment, and the ability to travel within a 50 mile radius on a daily basis.
Is responsible for adhering to all practice standards as they apply to patient care.

Qualifications
Education:
Graduate of an accredited school of nursing.
Certificates / Licenses:
Current state Registered Nurse license; or eligible for immediate state license.
Maintain Basic Life Support (BLS/CPR) Certification.
Maintain Advanced Cardiac Life Support (ACLS) Certification.
Current CCRN (Certification for Adult, Pediatric and Neonatal Critical Care Nurses) National Certification preferred.
Experience:
Able to meet physical and sensory requirements of job.
Able to successfully complete orientation of job performance.
Able to meet health standards for hospital employment.
Prior acute-care experience preferred, new graduates will be considered.
Must be highly motivated and committed to a quality of care that emphasizes the patient first.
